Telehealth Informed Consent

Prescripa LLC · Last updated September 22, 2026

This consent describes care delivered by telehealth through a storefront that runs on the Prescripa platform. Prescripa LLC, a Florida limited liability company (“Prescripa”), provides the technology and administrative services. Prescripa is not a medical provider and does not practice medicine. Your care is provided by the licensed medical practice responsible for it (the “Practice”): the brand’s own practice, or LB Medical Services, a separately owned professional association. The storefront names the Practice and your treating clinician before your visit.

What telehealth is

Telehealth uses secure video, audio, messaging, and online forms to evaluate and treat you without an in-person visit. The visit format depends on the rules of the state where you are located at the time of the visit. Where a state requires a live video visit, one takes place before any treatment.

Benefits and limits

  • Telehealth can make care easier to reach and faster to start.
  • A clinician cannot examine you in person. Information you do not provide, or that cannot be assessed remotely, may affect the evaluation.
  • Your clinician may decide that telehealth is not appropriate for you and recommend in-person care. Not every patient qualifies for treatment.
  • Technical problems can interrupt or delay a visit. Security safeguards are in place, but no electronic system is entirely free of risk.

Your clinician

Your treating clinician holds an active license in the state where you are located at the time of the visit and makes every clinical decision independently, including the decision not to treat. You will see your clinician’s name, credentials, and licensing state.

Recording

Where the Practice records telehealth visits, recordings are kept securely for clinical documentation, quality, safety, and regulatory compliance, and are shared only with your care team and as required by law. See the Privacy Policy.

Your consent

By continuing, you confirm that you are 18 or older, that you are the patient or the patient’s authorized representative, that you are located in a state where the service is available, and that the information you provide is accurate and complete. You consent to receive care by telehealth as described above. You may withdraw this consent for future visits at any time by writing to support@prescripa.com or to the Practice.
